Port Washington, NY - June 22, 2016 - Missing Persons Squad is investigating a Missing Person that occurred on Tuesday, June 21, 2016 at 3:00 pm in Port Washington.

According to detectives, Juan Rodriguez, 91, who resides at the Mayfair Nursing Home in Hempstead, was last seen at 3:00 pm when he walked out of the emergency room at St. Francis Hospital, 100 Port Washington Blvd.

Rodriguez has blue eyes, brown hair (balding), a slight beard and was wearing a black shirt and black pants. The missing suffers from dementia, has a heart condition and requires medication.
